This is a tuition subsidy form that must be filled out by your church parish in order to receive the subsidized tuition rate.

The Bishop's Scholarship Fund is granted through the Diocese of Lake Charles - Office of Catholic Schools. This fund provides aide for families in need for all schools in the Diocese of Lake Charles. Only current OLQHS families are eligible for this scholarship.
