Admissions Policies

Our Lady Queen of Heaven School’s admission process is designed to assess readiness of applicants while providing families and students an opportunity to explore the mission and goals of the school. 

Admission Requirements
 
Pre Kindergarten – Grade 1
  • Pre-K: A child must be four years of age on or before September 30th of the school year in which they plan to start.  
  • Submit an Online Application and $25 application fee
  • You must enter an email address for the child's current educational provider (daycare/school) during the online application process. An email will be sent directly to that teacher and they will be asked to fill out an online recommendation form.
    • This recommendation form must be completed in order for your application to be fully reviewed.
Grades 2-8
  • Submit an Online Application and $25 application fee
  • A copy of the student’s transcript including the most recent report card and any applicable standardized testing must be submitted.
  • These documents must be submitted online during the application process.
  • Upon receiving the Online Application, interview appointments will be scheduled for students in grades 4-8.
 
Copies of child(ren)’s birth certificate, immunization and sacramental certificates will be requested during the enrollment process, after the student is accepted.
